Programs & Forms

The following PDF forms must be download for your Bootcamp application.

Forms (1 & 2) below are required to be filled out and handed in to your instructor prior to starting your first session of your first Consort Fitness Bootcamp. You will be unable to participate in any Consort Fitness Bootcamp session’s until all paperwork is received and acknowledged.

1. Questionnaire
Providing accurate answers to all the questions below gives us in depth knowledge to your ability to be able to safely conduct fitness training, failure to disclose relevant information may put your safety at risk and exclude you from participating.

Please fax your completed form to Consort Fitness (03) 9789 6518 prior to your Bootcamp start date, alternatively you may bring the form to your first session.

Note: If you fail to complete and hand I your forms you will be unable to take part in the session.

2. Indemnity Form
The intent of this document is to enter into an agreement between the participant or their Parent / Guardian and Consort Fitness acknowledging the risks associated with participation in Consort Fitness activities and where applicable, includes a voluntary assumption of risk by participants, parents and legal guardians.

Please fax your completed form to Consort Fitness (03) 9789 6518 prior to your Bootcamp start date, alternatively you may bring the form to your first session.

Note: If you fail to complete and hand I your forms you will be unable to take part in the session.

3. Goals
This form will indicate to us your Goals & Training background which will enable us to cater for your desired goals and in turn help you achieve the best results possible.

Please fax your completed form to Consort Fitness (03) 9789 6518 prior to your Bootcamp start date, alternatively you may bring the form to your first session.

4. Feedback
Please review the instructors performance and return to Consort Fitness staff. Please feel free to write anything, this will only help the business to serve you better. We have a ‘steel’ exterior remember.

Please fax your completed form to Consort Fitness (03) 9789 6518 at the end of your Bootcamp, alternatively you may bring the form to your last session.

5. Direct Debit Form
The only contract within Consort Fitness, the Direct Debit option is available for a 26 or 52 weeks periods, Bank Debits and Credit Card payments. To apply for direct debit simply download the Direct Debit Form and fill in all the appropriate details from the guidelines below:

Direct Debit can be weekly, fortnightly or 4 weekly
Direct debit form must be received by Consort Fitness 5 working days prior to the debit commencement date.

Note: An early cancellation fee 4 weeks payment will apply for cancellations prior to the 26 and 52 week terms have been met. Direct Debits can be paused or canceled by Consort Fitness for certain circumstances eg, you suffer an injury, you move away for work etc.

Credit Card Payment

Credit Card payments can be for one off payments for example a payment of $229.00 for 1 Bootcamp, or if purchasing 2 or more Bootcamps or for continued Direct Debit payments for 26 or 52 weeks. To pay via a credit card payment simply download the Direct Debit Form, fill in the required information and return the form (preferably by fax) to Consort Fitness.

BC 1 - Albert Park 1730 - Program
Note: There is a proposed change to this program in the 3rd and 4th week moving 1 session to Sandringham. Your instructor will discuss this proposed change before week 3 and if members can't make the session it will be moved back to Albert Park.
